Patient arrested in theft of prescription forms at UF Health in Leesburg

A patient has been arrested in the theft of prescription forms from UF Health Leesburg Hospital.

A nurse practitioner at the hospital told Leesburg police that on Wednesday she had placed a zippered bag containing the prescription forms at the third floor nurses’ station. A witness saw a man later identified as 42-year-old James Wesley Park of Belleview carry the bag into the men’s restroom, according to an arrest report. When he came out of the restroom, the nurse practitioner followed him out of the building and called security. Her bag was recovered in the men’s restroom. Park was found to be in possession of the prescription forms.

Records showed that Park had been admitted to the hospital on Tuesday.

He was arrested on two counts of fraud and booked at the Lake County Jail on $4,000 bond.
